It's easy to obtain your mail-in rebate for selected Acuvue brand products, just follow the 6 steps below:

1. Download the rebate certificate at acuvue.com/rebates or call 1-888-565-8474 to have a certificate mailed to you. Please expect 1-2 weeks for delivery.
2. See your eye care professional for an eye exam, then purchase the specified amount Acuvue brand contact lenses.
3. Fill out the rebate certificate and attach the specified amount of Acuvue box tops.
4. Attach all receipts from both the eye exam and all product purchases to the rebate certificate.
5. Mail the completed rebate certificate, the required box tops and receipt(s) to the address specified on certificate. They must be mailed before the end date of the rebate promotion.
